Expand your abbreviations.
if you have 1000 milligrams, which is equal to 1 gram, spread over 1 square metre, what is it you want to convert?

You are asking for the number of milligrams per kilogram.
THat's just the same as trying to convert pounds (weight) per acre to pounds per ton. It doesn't make sense.

I assume this is for drug dosages, where some are specified as mg/kg (related to body mass), while some are specified as mg/m2 (related to "body surface area" or BSA).

So you need to find someone's BSA. There are several formulas, all of which take into account height in addition to mass. One of the easiest is Mosteller's formula.

BSA (m²) = ( [Height(cm) x Weight(kg) ]/ 3600 )½
e.g. BSA = SQRT( (cm*kg)/3600 )

or in inches and pounds:
BSA (m²) = ( [Height(in) x Weight(lbs) ]/ 3131 )½

I'm 6' 0" tall (183 cm) and weigh 175 lbs (80 kg). My BSA is 2.02.
My wife is 5'4" tall (163 cm) and weighs 125 lbs (57 kg). Her BSA is 1.6.

if you have 1000 mg/m2 and you want to give the dose based on mg/kg you divide the original amount by 30. For example 1000 mg/m2 is equal to 33.33 mg/kg. Hope that helps.
